help with Java Script and html home work 1

using HTML and JavaScript please create the following

 

 

Create a web form that has 3 text boxes, Firstname, Lastname and Age. Create a button that when clicked, populates a 4th textbox with the below

[Firstname] [Lastname] is [Number of Days]
